The hospice aide plays a crucial role in monitoring patients, documenting care, and reporting observations to ensure quality and safety in patient care.

KEY RESPONSIBILITIES:
The hospice aide's duties encompass, but are not limited to, the following:
  • Addressing personal care needs as assigned and responding promptly to patient requests.
  • Utilizing effective communication techniques with patients and families, ensuring confidentiality and respect.
  • Ensuring patient safety and proper use of equipment (e.g., foot stools, side rails, oxygen equipment).
  • Providing personal care services such as bathing, grooming, oral hygiene, and changing bed linens as needed.
  • Assisting patients with dressing and undressing as required.
  • Planning and preparing nutritious meals, including grocery shopping, as assigned.
  • Aiding in feeding patients as necessary.
  • Measuring and recording vital signs (temperature, pulse, respiration, blood pressure) as directed, demonstrating competency in these skills.
  • Monitoring and caring for patients' skin to prevent tissue breakdown.
  • Reporting significant changes in patients' conditions to the Case Manager and being aware of interpersonal dynamics affecting care.
  • Supporting patients in mobility and exercise as instructed by healthcare professionals.
  • Facilitating normal range of motion and assisting with transfers/positioning as directed.
  • Providing reminders for self-administered medications as permitted by state regulations.
  • Assisting with bedpans and urinals, and helping with light laundry tasks as assigned.
  • Performing range of motion exercises and other simple procedures as ordered, demonstrating competency.
  • Offering respite care for patients, families, and caregivers when on-site, as appropriate.
  • Maintaining cleanliness and orderliness in patients' living spaces as assigned.
  • Adhering to documentation standards and professional conduct guidelines.
  • Participating in quality improvement initiatives and team activities.
  • Respecting the personal needs and environment of patients and their families.

LIMITATIONS:
The hospice aide will not engage in activities considered nursing practices as defined by the Nurse Practice Act. Specifically, the aide will not administer medications, take physician orders, or perform procedures requiring nursing expertise.

QUALIFICATIONS:
  • Completion of state training requirements.
  • A minimum of 75 hours of classroom and supervised practical training, with 16 hours of classroom instruction completed prior to practical training.
  • Must be at least 18 years old.
  • Ability to read, follow written instructions, and document care provided.
  • Understanding of hospice philosophy and comfort in providing care to terminally ill patients.
  • Positive references from previous employers.
  • Self-directed with the ability to work independently, demonstrating composure in patient homes.
  • Empathy for the needs of the ill, injured, and frail individuals.
  • Current CPR certification is required.
  • Demonstrates patience, tact, and good personal hygiene.
  • Valid driver's license and insured vehicle in good working condition.
  • Successful completion of a probationary period to ensure competency in the above qualifications, documented by the supervisor.
