The Care Partner is a vital member of our healthcare team, working under the direction of the Registered Nurse to provide exceptional patient care and support. This role requires a strong focus on promoting a positive patient experience, partnering with the nurse, physician, and other healthcare providers to deliver excellence in patient care.
Key Responsibilities- Provide Basic Patient Care: Assist with daily living activities, comfort measures, and vital signs, ensuring a safe and comfortable environment for our patients.
- Support Clinical Care: Perform basic patient care functions, such as ADLs, ambulation, and vital signs, and assist with advanced technical training skills as required.
- Clerical Functions: Retrieve data, maintain unit supplies, and transport supplies/patient medications, ensuring efficient and effective care delivery.
- Excellence in Customer Service: Partner with healthcare team members, patients, and families to provide exceptional care and support, protecting patient rights and dignity at all times.
- Resident Centered Care: Provide resident care according to the written Plan of Care, ensuring basic resident care needs are met, and collaborating with the healthcare team to deliver high-quality care.
- Provide a Safe and Organized Environment: Perform basic housekeeping duties, clean and sanitize personal direct care items, and ensure safety hazards are removed or controlled.
- Emotional Support: Provide emotional support to residents and families, recognizing the value of life and promoting a peaceful presence.
- Education: High School Diploma or equivalent, or rising senior in good standing.
- Licenses and Certifications: Basic Life Support (BLS) certification, Georgia Driver's License (preferred), and Private Vehicle insurance (preferred).
- Experience: Less than 1 year of related experience.
- Skills: Ability to read, write, and speak English, basic computer skills, strong customer service and interpersonal skills, ability to collaborate with healthcare providers, and ability to function in stressful and emergency situations.
