Job Summary

This is a challenging and rewarding opportunity to join the Veterans Affairs, Veterans Health Administration team as a Registered Nurse in the Outpatient Mental Health Clinic. As a vital member of the Mental Health Team, you will work in collaboration with licensed independent providers to deliver high-quality care to Veterans with psychiatric diagnoses/mental health needs.

Responsibilities
  • Construct triage assessment plans to include reason for visit, vital signs, compliance with appointments, review of medications, pertinent lab work, medication and long-acting injection (LAI) administration (if applicable), clinical reminder activity, patient education, and action plan for Veteran.
  • Complete Same Day Evaluation Screenings for Veterans new or established to mental health services by ensuring access to care from any entry point across the facility.
  • Perform suicide risk assessment, depression screenings, and mental health-specific clinical reminders.
  • Act as liaison to the Emergency Department (ED) and Inpatient Psychiatry Unit by facilitating continuity of care, providing report for ED or inpatient admission.
  • Refer to, consult with, and coordinate care with the Behavioral Health Interdisciplinary Program (BHIP) teams and VA healthcare providers.
Requirements
  • U.S. Citizenship
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE)
