Job Summary

We are seeking a skilled and compassionate RN to join our Care Management team as an OP Care Management RN. This role will provide care management and population health services to patients, focusing on those with high vulnerability or stratified risk.

Key Responsibilities
  • Coordinate care around patients' chronic diseases, medication management, and transitions across the continuum.
  • Create personalized care plans for each patient, promoting effective partnerships between patients, families, nurses, physicians, and other healthcare disciplines.
  • Use data-driven approaches to manage patient populations, improving access to care and achieving optimal clinical outcomes.
  • Communicate effectively with patients and cross-continuum care team members to achieve optimal patient outcomes.
  • Document clinical, financial, and other information using appropriate systems.
  • Utilize data, dashboards, or reporting mechanisms to support high-reliability performance and outcome evaluation.
Requirements
  • College Diploma in Nursing
  • Registered Professional Nurse (RN) License in Illinois upon hire and Certified Case Manager within 2 years.
  • Three (3) years of clinical nursing experience with Case Management experience preferred.
Skills and Knowledge

The ideal candidate will possess strong clinical assessment and critical thinking skills, as well as effective behavioral and educational strategies, including motivational interviewing, teach-back method, and self-management support.

They will also demonstrate high-level professionalism, leadership, communication, education, and collaboration.
