Job Title: Home Health Nurse - LPN Position in Lakeland, Florida

About the Role:

  • Assist patients with transitioning home from the hospital and provide continuous care to remain at home with their loved ones.
  • Implement physicians' prescribed plans of care, conduct patient assessments, and coordinate care.
  • Administer prescribed medication, treatments, and therapies, and document care provided in a system of record.
  • Teach health promotion, and train family members on caring for their loved ones.

Location: Lakeland, Florida, and surrounding areas.

Salary and Benefits:

  • The estimated annual salary range for this position is $65,000-$85,000, depending on experience.

Responsibilities:

  • Accepting verbal orders from physicians and obtaining co-signature of Case Manager on all verbal orders.
  • Follow-up with physician, client, family members, staff, and Case Manager on client problems, concerns, and/or changes in client condition.
  • Perform other appropriate duties as assigned by management.

Qualifications:

  • An active, unencumbered license (RN/LPN/LVN) in the state of Florida.
  • Current CPR certification from American Red Cross, American Heart Association, or BLS.
  • TB skin test (current within last 12 months).
  • Six months prior hands-on nursing experience preferred but not required.
  • Reliable transportation is a must.
