Post Acute Care Specialist Job Summary:
We are currently seeking a highly motivated and detail-oriented Certified Nursing Assistant to join our rehabilitation services team at Terrebonne General Health System. The successful candidate will be responsible for providing exceptional patient care and support to our patients in a fast-paced clinical environment.

Key Responsibilities:
- Provide direct patient care under the supervision of licensed nurses, including vital signs, mobility assistance, and basic hygiene
- Develop and maintain effective relationships with patients, families, and healthcare teams to ensure seamless care coordination
- Collaborate with interdisciplinary teams to identify and address patient needs, promoting holistic care and patient satisfaction

Qualifications:
- Successful completion of a Nurse Aide Training Program or equivalent education in non-licensed nursing assistant duties
- Current certification as a Certified Nursing Assistant
- Excellent communication and interpersonal skills, with ability to work effectively in a dynamic team environment

Compensation:
