About the Role

We are seeking an experienced Palliative Care Nurse to join our team at Illumia Health, a fast-growing part of Gentiva. As a key member of our advanced illness management program, you will provide essential palliative care support to patients and their families during afterhours.

Key Responsibilities
  • Triage patient needs over the phone and respond appropriately to crisis calls from patients and families after hours
  • Make initial nursing evaluations in determining eligibility for palliative services within 48 hours of referral
  • Collaborate with patients, families, attending physicians, and other healthcare professionals to provide high-quality care
  • Maintain accurate and relevant clinical notes regarding patient condition and progress
About You

To be successful in this role, you will require:

  • Current RN license to practice in the state
  • Minimum 2 years of prior skilled Registered Nurse experience, preferably in palliative care, oncology, hospice, or geriatric care
  • Demonstrated ability to provide quality patient care and excellent communication skills
