Discharge Planner

We are seeking a skilled Discharge Planner to join our team at Pinnacle Treatment Centers KY-I LLC. As a Discharge Planner, you will play a critical role in creating and evaluating the discharge of clients from our treatment programs in a timely and efficient manner.

• Develop and implement discharge plans that meet the unique needs of each client, ensuring a smooth transition to the next level of care.
• Collaborate with Utilization Review staff to maximize reimbursement and maintain optimal census levels.
• Build and maintain relationships with referral sources and community agencies to ensure seamless transitions and optimal outcomes.
• Provide written discharge planning to patients and families, outlining the necessary steps for a successful transition.
• Accurately assess patients' financial resources and coordinate transitions to providers or other referral sources as needed.
• Document discharge plans in our Electronic Health Record (EHR) system, ensuring timely and accurate completion.
• Participate in multidisciplinary treatment management teams to ensure comprehensive care and optimal outcomes.
• Provide exceptional customer service to internal and external customers, responding promptly to inquiries and concerns.

• Bachelor's Degree from an accredited college or university in a related field, or applicable experience may be substituted for a degree.
• 3-5 years of clinical case management/discharge planning experience.
• Strong interpersonal and communication skills, with excellent verbal and written communication abilities.
