Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Trinity MedPro Staffing. As an LPN, you will be responsible for providing high-quality patient care in a long-term care facility setting.

Key Responsibilities
  • Medication Administration: Verify the identity of residents receiving medication, check the medication administration record (MAR) to ensure correct medication is being given, and record the time of administration or self-administration on specified forms or records.
  • Supervision of Medication Administration: Supervise the self-administration of medications by presenting medication to residents and observing ingestion or other application.
  • Vital Signs and Resident Care: Obtain and record residents' vital signs (temperature, blood pressure, pulse, and respiration rates), weight, food, and fluid intake and output, as directed.
  • Response to Medications: Observe residents to detect response to specified types of medications and prepare a report or notify designated personnel of unexpected reactions.
  • Call Signal Response: Answer residents' call signals to determine residents' needs within 7 minutes or less, according to facility standards.
  • Resident Safety and Hygiene: Maintain the safety of residents and ensure resident hygiene by following the bath schedule and providing baths and/or sponge baths as necessary.
  • Assistance with Daily Living: Assist residents with dressing and toileting as needed and assigned.
  • Incident Reporting: Observe residents for changes in condition and report any unusual occurrences to the Resident Services Director and/or supervisor in charge. Complete a complete incident report in its entirety for any unusual occurrence, event, or change in condition.
  • Meal Service and Cleaning: Assist with meal service by setting tables, serving residents, providing necessary refills or seconds, and removing dishes as residents are finished eating. Bus tables after meals to assist in cleaning the dining room. Vacuum rooms as assigned.
  • Compliance and Training: Complete daily paperwork as required to ensure all assigned duties are being performed for each resident. Complete all mandatory compliance, HIPAA, and state and federal regulatory training as required.
Requirements
  • A minimum of one (1) year of experience in a long-term care facility or hospital is required.
  • State license for LPN is required.
  • Negative TB test is required.
  • BLS certification is required.
  • Covid vaccination/waiver is required.
  • Good references are required.
