Medical Staff Professional

Kaiser Permanente is seeking a skilled Registered Nurse to join our team as a Case Manager Specialty RN (Bilingual). This role is a critical part of our healthcare team, working collaboratively with physicians and other healthcare professionals to manage patient care and ensure the best possible outcomes.
Key Responsibilities:- Assess patient needs and develop individualized care plans to address those needs.
- Work with physicians and other healthcare professionals to coordinate patient care and ensure seamless transitions between healthcare settings.
- Monitor patient progress and make adjustments to care plans as needed.
- Act as a patient advocate, educating patients and families about their conditions and empowering them to take an active role in their care.
- Develop and implement training and educational materials for healthcare staff and patients.
- Collaborate with external agencies and organizations to ensure patients receive the care they need.
- Minimum two years of clinical experience as an RN in an acute care or ambulatory care setting.
- Bachelor's degree in nursing or healthcare-related field.
- Case Management Certification or certification in the area of specialty preferred.
- Bilingual (English/Spanish) QBS Level II required.
