Home Health Registered Nurse Job Description

We are seeking a skilled and compassionate Home Health Registered Nurse to join our team at Interim HealthCare Staffing. As a Home Health Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities:
  • Participate in ongoing interdisciplinary assessment of patients to identify their unique needs and develop personalized care plans.
  • Conduct comprehensive assessments of patients' needs and develop plans of care in partnership with patients, representatives, and caregivers.
  • Provide ordered services by physicians as indicated in the plan of care, including counseling and education on healthcare-related needs.
  • Initiate preventative and rehabilitative nursing procedures as needed and refer patients to other services when necessary.
  • Monitor and evaluate patient outcomes, communicating changes in patient needs to the healthcare team.
  • Supervise paraprofessional staff and effectively communicate with other members of the interdisciplinary healthcare team.
Requirements:
  • Graduate of an accredited school of nursing with a current RN license in SC or compact license.
  • At least 2 years of experience as a nurse, with 1 year of experience in home health.
  • Proficient computer skills and knowledge of workplace software.
  • Ability to work with people, function well under stress, and use clinical judgment.
  • Clinical proficiency in nursing, strong communication skills, and compassion.
  • CPR certification and a valid driver's license with proof of insurance.
