Responsibilities
  • Conduct regular wellness visits to monitor residents' health and provide personalized care plans

Requirements

To be successful in this role, you'll need:

  • A current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN)
  • At least one year of experience in home health, assisted living, hospital, or long-term care environment
  • Excellent communication and interpersonal skills
  • Ability to work weekends, evenings, and flexible hours
  • Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel), and Sunrise applications
