Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Trinity Health. As a LPN, you will play a vital role in providing high-quality patient care in our outpatient programs.

Job Summary

The Licensed Practical Nurse will work under the supervision of a Registered Nurse to provide direct patient care, administer medications, and perform various clinical tasks. This is a dynamic and challenging role that requires strong communication and teamwork skills.

Responsibilities
  • Provide direct patient care, including vital signs, wound care, and medication administration.
  • Assist with procedures, such as phlebotomy and EKGs.
  • Document patient information using the EPIC system.
  • Collaborate with the interdisciplinary team to develop and implement patient care plans.
Requirements
  • Graduation from an accredited school for Licensed Practical Nursing and current licensure in the State of New York.
  • Maintains current BLS/CPR certification.
  • Participates in orientation and continuing education to stay up-to-date on clinical skills and knowledge.

This is a clinical setting with a fast-paced environment. The successful candidate will be able to work effectively in a team setting and prioritize tasks to meet the needs of patients and families.
