Job Description

We are looking for a caring and experienced professional to lead our care team and provide high-quality support to our residents. As a Care Manager, your primary responsibilities will include:

  • Developing and implementing individualized care plans that cater to the unique needs and preferences of each resident
  • Providing direct care and assistance with activities of daily living, such as bathing, dressing, and medication management
  • Monitoring resident progress and reporting any changes or concerns to management
  • Participating in interdepartmental collaboration to ensure seamless delivery of care services

You will be working closely with our skilled nursing staff, therapists, and other healthcare professionals to provide comprehensive and coordinated care. Requirements

To succeed in this role, you will need:

  • A high school diploma or equivalent
  • Previous experience working with seniors, preferably in a caregiving or healthcare setting
  • Excellent communication and interpersonal skills
  • The ability to work independently and collaboratively as part of a team
  • A commitment to providing high-quality care that respects the dignity and autonomy of each resident

In return for your dedication and expertise, we offer a competitive salary range of $55,000 - $65,000 per year, depending on experience and qualifications.
