PACT I Clinical Specialist Role

Bright Harbor Healthcare is seeking a highly skilled PACT I Clinical Specialist to join their multidisciplinary team. This role is responsible for facilitating the intake process, completing bio-psychosocial assessments, and managing treatment plans.
Responsibilities- Facilitate and complete intake process within 14 days of admission
- Develop and implement treatment plans that ensure timely and effective delivery of services
- Educate staff on treatment planning processes at weekly Team Treatment Planning meetings
- Assist consumers with activities of daily living through modeling, direct assistance, and coaching
- Master's Degree in a Human Services field
- LAC/LSW/LCAD/LPC preferred
- Two years' experience working with individuals with serious and persistent mental illness
- Competitive Salary: $65,000 - $80,000 per year
