We are seeking a skilled Speech Language Pathologist to join our team at Alternate Solutions Health Network LLC. As a Home Health Speech Language Pathologist, you will provide high-quality care to patients in the comfort of their own homes.

About the Role

As a Speech Language Pathologist, you will work with patients to improve their communication and swallowing skills. You will assess patients' needs, develop personalized treatment plans, and provide therapy sessions to help them achieve their goals.

Responsibilities
  • Provide home-based skilled speech therapy care for patients as directed by the attending physician
  • Guide patients in the use of therapeutic, creative, and self-care activities to improve functioning
  • Observe, record, and report patient reactions, changes, and progress to attending physicians and agency staff
  • Use activities and home exercise programs to improve swallowing, breathing, cognition, speech, and communication
  • Minimize patient risk by teaching preventative measures and promoting good decision-making
  • Counsel patients and families on disease management and provide education on speech therapy
