Care Alliance Behavioral Health Counselor Role

Job Summary: As a Behavioral Health Counselor at Care Alliance Health Center, you will be an integral part of our Patient-Centered Medical Home team. Your primary responsibility will be to address and reduce barriers to mental health and substance abuse care for our patients. This will involve diagnostic assessment, individual and group counseling, and patient advocacy for primary care patients experiencing emotional, behavioral, and substance abuse difficulties.

Key Responsibilities:

• Conduct comprehensive diagnostic assessments to identify patients' emotional and social needs.

• Develop and implement individualized treatment plans to address patients' mental health and substance abuse concerns.

• Provide ongoing therapeutic interventions for various stress/life problems, coping with medical issues, chronic disease, pain management, substance use, and mental health concerns.

• Collaborate with primary care teams to develop integrated care plans and ensure patients receive comprehensive care.

• Stay up-to-date with various behavioral health care models, including abstinence-only treatment, the disease model of addiction, and stages of change.

• Assess and respond to suicidal/homicidal risk, referring patients to relevant community resources as needed.

• Track clinical symptoms over time, identifying signs of exacerbation and decomposition of mental illness and substance use, and develop plans for management.

• Maintain accurate and timely documentation in the EMR, ensuring seamless communication with the integrated care team.

Requirements:

• Bachelor's degree in social work, counseling, or a behavioral health-related field.

• 3+ years of experience as a mental or behavioral health counselor.

• Valid LSW or LISW from the State of Ohio.

• Experience with therapeutic interventions, treatment planning, and trauma/substance use treatment interventions.

• FQHC and/or clinical experience a plus.
