Position Overview:

Key Responsibilities:
As a seasoned therapist, you will provide education to patients, their families, and caregivers, while collaborating with other healthcare professionals to resolve issues and meet individualized patient goals.

As part of the interprofessional care team, you will deliver direct patient care, ensuring compliance with safety protocols and guidelines to promote optimal patient safety, service quality, and successful outcomes.

This position also serves as a mentor and resource for less experienced staff, including other therapists and technicians, and may provide guidance to students and clinical fellows.

You will assist in the collection of quality metrics and analytics, contributing to the development of best practices and process improvement initiatives as assigned.

Essential Functions:
Contribute to teamwork by consistently offering support and responding positively to requests for assistance. Foster healthy relationships within the workplace and provide guidance to the interprofessional healthcare team as appropriate.

Utilize therapeutic communication techniques, including active listening and teaching, to build rapport with patients, families, and the healthcare team, collaborating on care plans.

Engage with team members by effectively communicating and reporting relevant patient care information and data comprehensively.

Educate nursing and support staff, orient new team members, and assist in the training of students during clinical rotations. Promote accountability among peers to enhance employee engagement scores.

Patient Care:
Conduct thorough evaluations of patients across a wide range of diagnoses and acuities to determine their functional levels and develop treatment plans accordingly.

Implement the physical therapy treatment plan based on evaluation findings, providing services to complex patient populations while respecting cultural diversity and inclusion principles.

Deliver education tailored to the learning needs of patients and their families, regularly assessing comprehension and adjusting teaching methods as necessary. Act as a resource for clinical fellows and nursing staff in best practices.

Collaborate with the healthcare team to modify treatment plans, track progress, and plan for discharge to achieve patient-specific goals.

Advocate for patients and families to address clinical and ethical concerns, utilizing appropriate resources.

Balance and adjust caseloads among team members to ensure optimal patient care and outcomes.

Initiate improvements in patient satisfaction scores through peer accountability to service standards.

Quality and Safety:
Regularly assess patients, monitor progress towards treatment goals, and modify plans as needed. Identify complications or limitations associated with procedures and proactively address them for optimal outcomes.

Provide oversight of physical therapy-related activities conducted by the interprofessional healthcare team, adjusting plans as necessary to enhance quality and maintain patient safety.

Identify opportunities for corrective action and contribute to process improvements by offering recommendations that align with departmental and hospital quality and safety targets.

Assist in developing treatment protocols and best practices based on research evidence to achieve patient safety outcomes, ensuring accurate documentation.

Documentation and Efficiency:
Maintain accurate and timely documentation of patient education and procedures performed in patient records. Ensure accurate charge entry for services rendered.

Utilize resources efficiently to maximize time and equipment usage. Manage time effectively, minimizing incidental overtime, and prioritize daily tasks while assisting coworkers as needed.

Professional Development:
Engage in innovative solutions through evidence-based practice and performance improvement projects. Actively participate in departmental projects and hospital committees.

Identify areas for innovation and improvement in patient care or unit operations, utilizing evidence-based literature. Collaborate with senior PTs on program development and clinical competencies.

Seek opportunities for self-development and expand learning beyond baseline competencies, focusing on continual growth as a Physical Therapist.

Ensure ongoing career discussions with management and maintain an updated development plan.
