Your daily responsibilities will include:
  • Engaging in meaningful one-on-one interactions with patients and their families to address daily needs
  • Conducting initial and ongoing assessments of patient and family requirements
  • Developing and implementing care plans based on comprehensive evaluations of medical histories
  • Documenting critical health metrics such as temperature, pulse, respiration, and blood pressure
  • Administering prescribed medications and treatments
  • Providing appropriate health care instructions to patients
  • Tracking changes in symptom management and outcomes
  • Completing and maintaining accurate clinical notes regarding patient conditions and care provided
  • Performing additional duties as assigned
Qualifications:
  • A genuine enthusiasm for delivering exceptional care to patients and their families
  • A valid Registered Nurse license in good standing
  • Preferred: 1 year of experience in hospice or home health settings
  • A valid driver's license
  • Current CPR certification
  • Strong observation, verbal, and written communication skills
  • Willingness to undertake additional related responsibilities as necessary
