About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Case Manager. As a key member of our healthcare team, you will be responsible for providing professional and non-professional nursing care services to patients, assessing, planning, implementing, and evaluating the care needs of a designated group of patients.

Key Responsibilities

  • Provide professional nursing care to patients, including assessing, planning, implementing, and evaluating the care needs of a designated group of patients.
  • Perform clinical nurse duties, including identifying clinical support tasks and providing nursing care and identified clinical support tasks within the scope of practice for achievement of the patient's plan of care as directed.
  • Collaborate with the healthcare team to ensure seamless care delivery and patient satisfaction.
  • Manage patient flow and prioritize tasks to ensure efficient care delivery.
  • Develop and maintain effective relationships with patients, families, and healthcare team members.

Requirements

  • Registered Nurse (RN) license or license eligible.
  • Basic Life Support (BLS) certification.
  • Three (3) years of Case Management experience.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN)
  • Specialty Nursing certifications.
