- Assists patients with personal care tasks as outlined in their care plan, including hygiene, mobility, and elimination needs. Reports any inability to perform tasks to the RN or Director of Clinical Services promptly.
- Ensures a safe environment for patients by using equipment correctly and safely (e.g., footstools, side rails, oxygen).
- Adheres to Universal Infection Control Precautions during all patient interactions.
- Helps patients and families with home maintenance tasks as specified in the care plan, such as light cleaning and meal preparation.
- Monitors and records vital signs and basic bodily functions, ensuring adequate fluid and nutrition intake.
- Utilizes safe techniques for transferring and assisting patients with mobility.
- Identifies when intervention from other home health team members is necessary and communicates this to the RN or Director of Clinical Services.
- Respects the privacy and confidentiality rights of all patients and their families.
- Documents care provided and patient responses accurately and submits reports in a timely manner.
- Operates within the scope of training and experience.
- Complies with all relevant local, state, and federal regulations, including Medicare guidelines and agency policies.
- Manages a caseload effectively while delivering care in a cost-efficient manner.
- Consistently meets or surpasses the service standards set by 365 Health Services.
- Conducts all professional activities ethically and with integrity.
- Utilizes supplies and equipment in an effective and efficient manner.
- Completes a minimum of 12 hours of in-service training annually.
- Maintains consistent attendance.
- Adheres to the Code of Conduct established by 365 Health Services.
- Engages positively with all staff, supporting the mission and vision of 365 Health Services.
- Ensures compliance with general orientation and agency policies, including confidentiality and HIPAA regulations.
