Key Responsibilities:

  • Triaging and Coordination: Respond to triage needs, complete direct referral acceptance processes, and utilize technology to review patient clinical needs.
  • Clinical Determination: Provide timely clinical determinations for admission, working closely with referral sources, physicians, and other healthcare professionals.
  • Communication and Collaboration: Maintain effective communication with referral sources, patients, and families, ensuring seamless coordination of transfers.
  • Documentation and Reporting: Accurately document clinical referrals, maintain program and patient information, and create reports to analyze efficiency.
  • Qualifications:
    • Valid LVN License: Current licensure as a Licensed Vocational Nurse in the state of Texas.
    • Certifications: Completion of Crisis Prevention Intervention (CPI) Certification/Training and BLS (CPR) through the American Heart Association.
    • Language Skills: Ability to read, write, and communicate effectively in English.
