Role Overview:


The Nurse Practitioner at Oak Street Health is dedicated to delivering effective and equitable primary care to Medicare recipients, ensuring their well-being and minimizing hospital visits.

Nurse Practitioners operate autonomously while collaborating with physicians based on individual patient needs.

They provide ongoing care and foster strong patient relationships, which cultivates trust and enhances patient engagement with their care plans.


Nurse Practitioners are integral members of our outpatient care teams, which include physicians, other nurse practitioners or physician assistants, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers.

We collaborate with a network of top-tier specialists and hospitals for additional care.

This allows our healthcare professionals to concentrate on in-clinic services:
primary care, care coordination, and population health.

Key Responsibilities:

  • Deliver outstanding primary care services.
  • Conduct office consultations for both routine and urgent health concerns.
  • Perform annual wellness assessments and health risk evaluations, requiring a comprehensive understanding of health and precise documentation.
  • Coordinate care with other healthcare providers, specialists, testing facilities, and agencies.
  • Lead population health initiatives in collaboration with the Care Team (e.g., ensuring eligible females receive their biannual breast cancer screenings).
  • Assist the care team with phone triage and outreach efforts.
  • Educate patients regarding their health conditions, treatment plans, and care options.
  • Engage in promotional activities for Oak Street Health.
  • Conduct home visits as necessary.
  • Perform additional duties as assigned.
This position reports to the Center Medical Director and collaborates closely with operational leadership.

Required Qualifications:

  • Master of Science in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program
  • National certification as a Nurse Practitioner through AANP or ANCC
  • Active, non-probationary state Nurse Practitioner license
  • Active DEA license
  • Authorization to work in the US

  • Experience in primary care settings such as internal medicine, geriatrics, or family medicine
  • Familiarity with HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments (HRAs).

  • Interest in being part of an innovative model focused on evidence-based population health
  • Bilingual capabilities in relevant languages
