Nurse Practitioner Opportunity

We are seeking a skilled Nurse Practitioner to join our team at Matrix Medical Network. This is a PRN/Part Time opportunity that offers flexible scheduling and the chance to make a direct impact on the health and well-being of others.
About the RoleThis role involves conducting comprehensive health assessments in patients' homes, where you will review and observe their current health, medical history, medication adherence, social environment, and other risks. Your findings will provide invaluable insight into their overall health and well-being, which can be challenging to capture during routine office visits.
As a Nurse Practitioner with Matrix Medical Network, you will have the opportunity to work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as needed. You will also collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up care.
Responsibilities- Conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols
- Work collaboratively with healthcare professionals to provide comprehensive care
- Collaborate with PCPs on patient education and provide follow-up care
- Provide services in various venues, including home visits, skilled facility visits, and telehealth visits
- Master's Degree or equivalent experience and satisfactory completion of NP licensure required
- Current RN and NP licensure in state of practice, including prescription authority or ability to obtain prescriptive authority
- Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
- Current BLS, ACLS, or CPR certification
- Minimum 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health records
- Excellent verbal and written communication skills with patients, clients, and colleagues
$90.00 per completed assessment, paid bi-weekly. Flexible scheduling and mileage reimbursement plan available.
