Compensation: The estimated salary for this position is $75,000 - $90,000 per year, depending on experience.

About This Role

We are seeking a skilled and compassionate Patient Care Specialist to join our outpatient physical therapy team. As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to provide high-quality care and achieve optimal outcomes.

Main Responsibilities

  • Evaluate and develop individualized treatment plans based on patient needs and goals.
  • Administer and interpret tests and measurements to assess patient progress.
  • Review patient charts and adjust treatment plans as needed.
  • Develop functional goals with patients and/or family members to promote independence and improve quality of life.
  • Collaborate with physicians and other healthcare professionals to ensure comprehensive care.

Requirements

  • Masters Degree in Physical Therapy or Doctorate in Physical Therapy required.
  • Current CLT certification or > 3 years outpatient PT experience preferred.
  • State licensure required.
