Job Summary

The Physical Therapist (PT) is a key member of our team, responsible for evaluating and assessing patient needs, developing treatment plans, and providing therapy services in a home care environment. The ideal candidate will have a strong background in physical therapy, excellent communication skills, and the ability to work collaboratively with physicians and case managers.

Responsibilities
  • Evaluate and assess patient needs to develop effective treatment plans

  • Work cooperatively with physicians and case managers to ensure seamless care
  • Utilize creativity and latitude to provide high-quality care
Requirements
  • Graduate of a Baccalaureate, Master's, or Doctorate program in Physical Therapy
  • Current and unencumbered licensure to practice as a Physical Therapist in the state of employment
  • Two years of experience as a Physical Therapist, with community/home health experience preferred
  • Excellent verbal and written communication skills
  • Access to reliable transportation
