Role Summary:
The Physical Therapist is responsible for assessing, preventing, and treating physical dysfunction or pain. This role involves evaluating patients and implementing therapeutic interventions, utilizing physical and chemical modalities, exercise, and education for patients and their families. The Physical Therapist collaborates with patients, their representatives, and caregivers to develop and maintain individualized care plans, ensuring effective communication with the healthcare team.
Key Responsibilities:
1. Conduct comprehensive physical therapy evaluations, assessing functional limitations and mobility status.
2. Identify additional home health service needs for patients and facilitate referrals as necessary.
3. Deliver prescribed services as outlined in the care plan, maintaining communication with the healthcare team.
4. Administer appropriate treatments aimed at alleviating pain, restoring function, and enhancing independence through the use of suitable equipment and techniques.
5. Document evaluation findings, treatment progress, and discharge planning on a visit basis.
6. Engage in interdisciplinary case management discussions to ensure coordinated care.
7. Provide education and counseling to patients, families, and healthcare staff regarding treatment plans and document these interactions.
8. Arrange for necessary equipment to support patient independence and functionality.
9. Participate in quality assurance initiatives and ongoing training programs.
10. Supervise the work of Physical Therapist Assistants and home health aides as needed.
11. Adhere to all compliance requirements associated with the role.
12. Perform additional duties as assigned.
Additional Information:
Required Qualifications:
- Minimum of one year of relevant experience.
Preferred Qualifications:
- Experience in a recognized home health agency.
Work Environment:
- Full-Time position with standard hours of 40.00 per week.
- Day shift, Monday through Friday, with weekend rotation as required.
Credentials:
Essential:
- Licensed Physical Therapist.
Education:
Essential:
- Bachelor's Degree in a relevant field.
Working Conditions:
Essential:
- Requires significant manual dexterity and motor coordination.
- Involves substantial physical effort, potentially exceeding 125 lbs. of force.
- May encounter exposure to various environmental factors.
