Nurse Practitioner Role Overview

At Abode Care Partners, we are seeking a dedicated Nurse Practitioner (APRN) to deliver high-quality medical care to seniors and individuals with complex conditions in various settings.

This role involves providing evidence-based medical care, managing patient needs, and collaborating with healthcare professionals to ensure comprehensive patient health. As a Nurse Practitioner at Abode Care Partners, you will play a vital role in enhancing the lives of our patients.

Responsibilities:
  • Medical Care Provision: Deliver evidence-based medical care, including examinations, treatments, and laboratory testing as needed.
  • Patient Management: Manage patient needs, ensuring personalized care and addressing chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
  • Collaboration: Work closely with caregivers and other healthcare professionals to provide interdisciplinary care and maintain a comprehensive plan of care.
  • Referrals: Provide referrals to other healthcare practitioners when necessary.
  • Medical Records: Ensure patient medical records are kept up-to-date.
Qualifications:
  • Licensure: Hold an active state Nurse Practitioners license, Current Nurse Practitioner National Board Certification, and Active state DEA licensure.
  • Education: Possess a Master's degree in Nursing (MSN).
  • Experience: Have five or more years of nursing experience, with at least two years of nurse practitioner experience preferred.
  • Skills: Demonstrate strong interpersonal communication skills, excellent time management skills, proficiency in medical instruments and equipment, and knowledge of HIPAA and OSHA regulations.
