About the Role

We are seeking a skilled LPN to provide high-quality nursing care to our patients in post-acute facilities in the Euclid, Ohio area. As an LPN with IntelyCare, you will have the opportunity to work with a variety of patients, develop your skills, and advance your career.

Responsibilities
  • Provide compassionate nursing care to patients in post-acute facilities
  • Monitor patients' physical and mental conditions and track vital signs
  • Perform resident intakes, document medical histories, and track health concerns
  • Administer and verify medications, track use and effects
  • Cultivate a comfortable and safe facility for residents
  • Track medical records and charts
  • Assist with IVs, immunizations, and phlebotomy procedures
  • Manage medical supplies and maintain inventory
  • Follow and adhere to resident care plans
  • Work with staff to orient and train new colleagues
Requirements
  • Current Ohio state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved LPN educational program
  • High school graduation, GED, or equivalent
  • At least 6 months of experience (LPN or similar role)
  • Ability to lift a minimum of 50 lbs
