Position Overview: The Complex Case Coordination (C3) Liaison plays a crucial role in responding to urgent referrals from warriors in crisis. This position involves conducting initial assessments, guiding veterans to appropriate resources, and managing treatment plans to ensure effective stabilization and care.
Key Responsibilities:
- Caseload Management: Maintain a manageable caseload of assigned warriors, serving as a subject-matter expert in mental health program coordination.
- Assessment and Planning: Conduct comprehensive screenings to identify the needs of warriors facing complex health challenges, and collaborate with care teams to develop tailored treatment plans.
- Advocacy: Act as an advocate for warriors, ensuring they receive the necessary support and resources throughout their recovery journey.
- Documentation: Accurately maintain case documentation and sensitive health information in compliance with organizational standards.
- Program Coordination: Facilitate comprehensive support by coordinating with internal programs and external organizations, including governmental agencies and community resources.
Qualifications:
- Four years of experience in social work or a related mental health field.
- Experience working with veteran healthcare programs and case management.
- Bachelor's degree in social work, psychology, or a related field.
