Job Overview

Hospice of the Western Reserve is seeking a compassionate and skilled nurse to join our team as an After Hours Nurse. In this role, you will provide critical care to patients and families in their homes, utilizing your nursing skills to make a meaningful difference in their lives.

Key Responsibilities:

  • Assess and address the physical, psychosocial, and spiritual needs of patients and families.
  • Provide skilled nursing services, including pain and symptom management, under the direction of the patient's physician.
  • Monitor and evaluate patient progress, making adjustments to care plans as needed.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in ongoing education and training to enhance your skills and knowledge.

Requirements:

  • Graduate of an accredited school of nursing, with a BSN preferred.
  • Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Ohio.
  • Experience in pain and symptom management, oncology, or mental health is preferred.
  • Excellent communication and interpersonal skills, with the ability to work independently and collaboratively as part of a team.
  • Ability to access patients and families in various settings, including homes and public areas, in any weather condition.
