RN Case Manager

As a RN Case Manager - Home Care Specialist with Interim HealthCare of the Twin Cities, you will have the opportunity to work independently and autonomously in a home environment, visiting patients one on one to provide high-quality care.
Key Responsibilities:
- Perform assessments and procedures to evaluate patient needs and develop individualized care plans.
- Develop working relationships with discharge planners and social service workers to ensure seamless transitions of care.
- Assess and review the matching of employee skills to client needs to ensure optimal care delivery.
- Visit clients in their homes to evaluate the quality of care being provided and review clinical notes.
- Participate in ongoing interdisciplinary assessment of the patient to ensure comprehensive care.
Requirements:
- Graduate of an accredited school of nursing with a current license as a Registered Nurse in the State of MN.
- Able to travel within a geographic area serviced by the office with reliable transportation, a valid driver's license, and proof of auto insurance.
- CPR Certification.
- Two (2) years of current nursing experience.
- One (1) year case management experience preferred.
