In the role of Clinical Assessment Nurse, you will play a vital role in equipping your team with the expertise required to address each resident's distinct needs. Your contributions will guarantee that our residents receive the exceptional care they expect from our facility by creating, monitoring, auditing, and adjusting each resident's care plan to meet their individual requirements and aspirations. This includes conducting resident evaluations and supporting the discharge process. Our residents will rely on your expertise, skills, and meticulous attention to detail to ensure their comfort and safety.

Qualifications:

To be considered for this position, applicants must possess:

  • An unencumbered State of Illinois R.N. License.
  • A degree from an accredited nursing program.
  • C.P.R. Certification.
  • Preferably, prior experience as a MDS Coordinator.
  • At least one (1) year of experience as an R.N. in a Skilled Nursing Facility.
  • One (1) year of experience as a C.N.A.
