Occupational Therapist

Ascension is seeking a skilled Licensed Occupational Therapist to join our Home Health team. As an Occupational Therapist, you will play a vital role in helping patients recover from injuries or illnesses and regain their independence.
Responsibilities
Evaluate and treat patients with physical, emotional, cognitive, and/or psychosocial deficits
Assess patients' ability to safely and effectively perform independent living skills
Design and implement individualized plans of care, including type and frequency of treatment
Document patient progress and modify plans as needed
Educate patients and families on patient deficits and provide information on adaptive equipment and support programs
Requirements
Bachelor's degree in Occupational Therapy required; Master's degree preferred
Current licensure as an Occupational Therapist in the state of Illinois
BLS Provider certification within 1 month of hire date
Drivers License and Rehab Therapist specializing in Occupational Therapy (OTR) credentials from the Illinois Department of Financial and Professional Regulation
