Nutrition Specialist

We are seeking a skilled Nutrition Specialist to join our team at Concordia Lutheran Ministries and Affiliates. As a Nutrition Specialist, you will play a vital role in improving the diet, nutrition, and health of our home health patients.

The successful candidate will provide nutritional counseling, education, and support to patients with medical problems affected by nutrition status. You will work closely with our clinical team to develop and implement individualized diet therapy plans, conduct nutritional screenings and assessments, and provide in-service educational programs for clinicians.

• Provide nutritional counseling and education to patients and their families
• Develop and implement individualized diet therapy plans
• Conduct nutritional screenings and assessments for assigned patients
• Act as a nutrition resource person on the healthcare treatment team
• Plan and implement in-service educational programs for clinicians
• Maintain patient and staff confidentiality
• Perform related work as required

• Graduation from an accredited college or university with an advanced degree in nutrition or its equivalent
• Ability to communicate ideas effectively, both orally and in writing
• Thorough knowledge of nutritional care and counseling principles and practices
• Ability to identify nutrition education needs and develop an appropriate program to meet individual patient needs
• Ability to maintain favorable public relations
• Current CPR certification
• Reliable, independent means of transportation to make home visits
