Job Summary

We are seeking a highly skilled Registered Nurse Case Manager to join our team at AccentCare, Inc. as a Home Health Specialist. As a key member of our clinical team, you will be responsible for providing exceptional patient care and case management services to our patients in the comfort of their own homes.

Key Responsibilities
  • Conduct thorough assessments of patients' medical needs and develop personalized care plans to ensure optimal health outcomes.
  • Collaborate with healthcare providers, family members, and caregivers to coordinate patient care and services.
  • Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle habits.
  • Monitor patients' progress and adjust care plans as needed to ensure timely and effective interventions.
  • Document all patient interactions, assessments, and care plans in a timely and accurate manner.
Requirements
  • Graduate from an accredited school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
  • One (1) year experience as a RN in a home health or case management setting.
  • Required certifications and licensures: licensed to practice as a registered nurse in the state of agency operation, valid CPR certification, and a valid driver's license with an automobile that is insured in accordance with state and/or organization requirements.
