We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Field Staff member in Home Health and Hospice care. As an RN Field Staff, you will provide high-quality patient care in the comfort of patients' homes, working independently and as part of a collaborative team.
Key Responsibilities- Provide ongoing assessment and nursing care to patients in their homes, developing personalized care plans to meet their unique needs.
- Deliver home health care, hospice care, or both, working closely with patients, families, and healthcare teams to ensure seamless care transitions.
- Maintain accurate patient records through the use of mobile devices, ensuring timely and efficient documentation.
- Collaborate with interdisciplinary teams to coordinate patient care, addressing any concerns or issues that may arise.
- Current Ohio nursing license and at least one year of acute care or medical-surgical experience.
- Experience in a Medicare-certified home health agency and knowledge of Electronic Medical Records (EMR) systems are preferred.
- Valid driver's license, insurance, and an acceptable driving record.
