About the Role

We are seeking a highly skilled and compassionate Primary Care Physician to join our team at CenterWell Senior Primary Care. As a key member of our care team, you will play a vital role in delivering high-quality, patient-centered care to our senior patients.

Responsibilities
  • Evaluate and treat patients in accordance with standards of care, utilizing your expertise in family medicine, internal medicine, or geriatrics.
  • Collaborate with our interdisciplinary care team to coordinate patient services, including specialty referrals, hospital and SNF coordination, and durable medical equipment.
  • Act as a key source of medical expertise, participating in daily huddles and contributing to the development of care plans that prioritize patient well-being.
  • Complete all medical record documentation in a timely manner, working with our quality-based coder to optimize coding specificity.
  • Stay up-to-date with policy and protocol defined by Clinical Leadership, ensuring compliance with licensing, certification, and accrediting bodies.
  • Participate in growth opportunities for new or existing services within the Center, driving innovation and excellence in patient care.
Requirements
  • Current and unrestricted medical license or willingness to obtain a medical license in the state of practice.
  • Graduate of an accredited MD or DO program.
  • Excellent verbal and written communication skills, with a high level of skill in interpersonal relationships and communications with colleagues and patients.
  • Fully engaged in the concept of integrated team-based care, with a willingness to learn and adapt to practice in a value-based care setting.
  • Superior patient/customer service skills, with basic computer skills, including email and EMR.
