Position Overview:

The Hospice RN Case Manager will be responsible for delivering exceptional care to patients in designated areas. This role requires a commitment to maintaining the highest standards of patient management and care coordination.

Key Responsibilities:

  • Report to the Administrator or Patient Care Manager, overseeing the management of each patient’s care.
  • Serve as the primary point of contact between the physician, patient, family, and hospice team.
  • Deliver direct care services to hospice patients while providing emotional support to their families.
  • Educate caregivers on effective care practices for their loved ones.
  • Collaborate with an interdisciplinary team, including LPNs, CNAs, and support staff, to ensure comprehensive patient care.
  • Assess patient and family needs, contributing to the development and implementation of personalized care plans.
  • Maintain accurate clinical records for all assigned patients.
  • Engage in quality assessment and performance improvement initiatives.

Qualifications:

  • Compassionate and empathetic approach to patient care.
  • Ability to provide care in various home settings.
  • Demonstrated resilience and calmness in crisis situations.
  • Current RN license in the state of employment or eligibility to obtain.
  • Minimum of 1 year of nursing experience in hospice, home health, or related fields preferred.
  • Additional experience in hospital settings, critical care, or emergency services is advantageous.
  • Valid driver's license and automobile insurance.
  • Current CPR certification.
