Position Overview:
The Hospice RN Case Manager will be responsible for delivering exceptional care to patients in designated areas. This role requires a commitment to maintaining the highest standards of patient management and care coordination.
Key Responsibilities:
- Report to the Administrator or Patient Care Manager, overseeing the management of each patient’s care.
- Serve as the primary point of contact between the physician, patient, family, and hospice team.
- Deliver direct care services to hospice patients while providing emotional support to their families.
- Educate caregivers on effective care practices for their loved ones.
- Collaborate with an interdisciplinary team, including LPNs, CNAs, and support staff, to ensure comprehensive patient care.
- Assess patient and family needs, contributing to the development and implementation of personalized care plans.
- Maintain accurate clinical records for all assigned patients.
- Engage in quality assessment and performance improvement initiatives.
Qualifications:
- Compassionate and empathetic approach to patient care.
- Ability to provide care in various home settings.
- Demonstrated resilience and calmness in crisis situations.
- Current RN license in the state of employment or eligibility to obtain.
- Minimum of 1 year of nursing experience in hospice, home health, or related fields preferred.
- Additional experience in hospital settings, critical care, or emergency services is advantageous.
- Valid driver's license and automobile insurance.
- Current CPR certification.
