Job Summary

Agrace HospiceCare is seeking a compassionate and skilled Certified Nursing Assistant to join our team as a Homecare CNA. In this role, you will provide high-quality patient care and support to individuals in their homes, promoting a comfortable and dignified end-of-life experience.

Main Responsibilities

  1. Assist patients with activities of daily living, including bathing, dressing, and grooming
  2. Provide homemaker assistance, such as light housekeeping and meal preparation
  3. Communicate effectively with patients, families, and healthcare teams, documenting care and changes in patient condition
  4. Collaborate with RN Case Managers and Social Workers to develop and implement patient care plans

Requirements

  • Current, unrestricted Nursing Assistant Certification in the state of employment
  • One year of personal care experience, hospice, or home health experience preferred
  • Valid driver's license, auto insurance, and access to a reliable vehicle
  • Ability to develop, maintain, and apply knowledge of hospice and end-of-life process
  • Ability to maintain professional boundaries with patients, families, and co-workers
