Job Summary

As a Case Manager, you will provide professional nursing care for patients in their homes or alternate-site settings. You will work closely with patients, families, and healthcare teams to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities
  • Plan, provide, and document professional nursing care for patients in accordance with the NC nurse practice act, physician orders, and established policies and procedures.
  • Use professional nursing judgment to individualize the plan of care based on assessment of the patient's baseline needs and response to care.
  • Collaborate with healthcare teams to evaluate and cost-effectively provide patient care.
  • Assess educational needs of patients/families and develop effective discharge plans.
  • Role model professional nursing through conduct, appearance, communication, mutual respect, ethical decision-making, critical thinking, and problem-solving skills.
Requirements
  • Associate Degree or Diploma in Nursing required.
  • Current North Carolina RN Licensure or compact RN licensure in participating state.
  • Current State of North Carolina Cardio-Pulmonary Resuscitation Certificate (CPR) (American Heart Association only).
  • Two years of med-surgical, critical care, or home health/hospice/infusion experience.
