Job Summary:The Registered Nurse will be responsible for coordinating and overseeing the nursing process for residents assigned to their care. This includes providing direct and indirect care according to each resident's nursing and medical plan of care, as well as adhering to state and federal regulations and accepted nursing standards of practice.
Key Responsibilities:- Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination.
- Develop and implement individualized care plans that meet the unique needs of each resident.
- Provide direct care to residents, including administering medications, performing assessments, and implementing interventions as needed.
- Collaborate with other healthcare professionals to ensure comprehensive care and address any concerns or issues that may arise.
- Document resident care and progress in a timely and accurate manner.
- Participate in ongoing education and training to stay current on best practices and industry developments.
- Associate's Degree in Nursing, Bachelor's preferred.
- State Certified RN license.
- 1 year professional nurse experience.
- Experience in Certified Home Care and OASIS.
- Valid driver's license and reliable transportation.
