Job Summary

This role focuses on transition planning and Utilization Review, working closely with a Care Team to facilitate throughput. The ideal candidate will possess good critical thinking skills and a willingness to learn in a fast-paced environment.

Responsibilities
  • Initial assessment and development of transition plans
  • Participation in multidisciplinary rounds
  • Utilization review
  • Patient advocacy
  • Ability to delegate tasks to Case Management assistants
Requirements
  • High degree of skill in written/oral communications and personal interactions
  • Demonstrates agility in a continuously changing fast-paced work environment
  • Demonstrates computer competency
  • Ability to complete daily job responsibilities within normal hours
  • Culturally competent
  • Three or more years of clinical experience in a healthcare delivery system
  • Completion of an accredited Nursing program
  • Current Nursing License in the State of MA
