About the Position

Optimal Home Care is seeking a licensed Physical Therapist to provide high-quality patient care services in the comfort of our clients' homes.

Key Responsibilities:
  • Assess patients and develop personalized treatment plans with measurable goals and time frames.
  • Comply with ethical and professional standards of conduct as set forth by the American Physical Therapy Association.
  • Demonstrate treatment skills rooted in evidence-based practice guidelines, balancing patient and provider expectations and needs.
Requirements:
  • Graduate of a physical therapist education program approved by the Commission on Accreditation in Physical Therapy Education (CAPTE) or a successor organization of CAPTE, or a school approved by the American Medical Association (AMA).
  • At least one (1) year of experience as a Physical Therapist.
  • Currently licensed and in good standing in the state of Colorado.
  • Ability to communicate in the language of the client and document client-related activity as appropriate.
  • Adequate and effective written and verbal communication skills.
  • Adequate and effective interpersonal skills.
