Job Title: Travel Nurse RN Case Manager

Solomon Page is seeking a skilled Travel Nurse RN Case Manager to join our team in Springfield, Missouri.

Job Summary:

We are looking for a highly motivated and experienced Registered Nurse to provide case management services to our clients. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work independently and as part of a team.

Responsibilities:
  • Provide case management services to clients, including assessment, planning, implementation, and evaluation of care.
  • Develop and implement individualized care plans that meet the unique needs of each client.
  • Collaborate with healthcare providers, family members, and other stakeholders to ensure seamless care coordination.
  • Conduct regular assessments and evaluations to ensure the effectiveness of care plans.
  • Document all interactions and interventions in a timely and accurate manner.
Requirements:
  • Current Missouri license as a Registered Nurse.
  • Excellent interpersonal and communication skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
  • Ability to stand for long periods of time.
