Job Description

We are seeking a skilled Discharge Planner RN to join our team at Providence Regional Medical Center Everett. This is an exciting opportunity for a dedicated professional to make a meaningful impact in the lives of our patients and their families.

About the Role

The Discharge Planner RN will play a vital role in coordinating activities related to transition planning, developing and implementing care plans that meet the needs of our patients, and ensuring continuity of care. This position requires collaboration with medical staff, primary care physicians, patients, families, community agencies, and facilities involved in patient care.

The ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. Experience in discharge planning for medical and surgical patients in an inpatient or outpatient setting is highly desirable.

Key Responsibilities
  • Assess patient needs and develop individualized care plans
  • Coordinate with medical staff, primary care physicians, and other healthcare professionals to ensure seamless transitions
  • Communicate effectively with patients, families, and caregivers to provide support and education
  • Collaborate with community agencies and facilities to identify resources and services that meet patient needs
Required Qualifications

To be considered for this role, you must have:

  • A nursing degree or diploma upon hire (or equivalent education/experience)
  • A valid Washington Registered Nurse License
  • Experience in discharge planning for medical and/or surgical patients in an inpatient or outpatient setting
Preferred Qualifications

Prior experience in an outpatient clinic setting providing primary or specialty care, or in an acute hospital setting doing discharge planning, is highly valued.
