Job Summary:

We are seeking a skilled Clinical Chaplain to provide spiritual support and counseling to home care patients, families, and staff in a culturally inclusive manner. As a member of the interdisciplinary patient care team, you will participate in patient care planning, document interventions, and meet regulatory guidelines.

Essential Responsibilities:
  • Administrative Duties:
    • Participate in Quality Assurance Activities, including committees for Patient Satisfaction Scores, QI Projects, and UBT.
    • Conduct In-services and CEU programs to improve Integrated Model of Care.
    • Provide Staff support to maintain Spiritual Resilience in Healthcare.
  • Direct Patient Care:
    • Provide spiritual support and counseling to home care patients, families, and staff in a culturally competent manner.
    • Participate in creating patient care plans, including acuity mapping and documenting interventions.
    • Support bereavement care plans for surviving family members.
Basic Qualifications:
  • Minimum of one (1) year of experience as a Clinical Chaplain/Spiritual Counselor in a Hospital, Ambulatory, Home Health, Palliative Care, or Hospice setting.
  • Masters Degree in Divinity, Theology, or related field from an accredited Seminary, College, or University.
  • Driver's License (California) and Basic Life Support required at hire.
Additional Requirements:
  • Ability to serve and support people of all religions, world views, and spiritual perspectives.
  • Maintain adequate automobile liability insurance as required by Hospice Program policy.
  • Meet Medical Center/Homecare requirements for health appraisal and annual PPD testing.
Preferred Qualifications:
  • Two (2) or more years of experience as a Clinical Chaplain/Spiritual Counselor in a Hospital, Home Health, Palliative, or Hospice Setting.
  • Board Certified Chaplain Certification from Association of Professional Chaplains.
