Job SummaryWe're seeking compassionate Registered Nurses with a passion for hospice care to join our extraordinary care team. As an After Hours Hospice Registered Nurse, you'll provide management and delivery of patient care plans, working closely with patients, families, and healthcare teams to achieve optimal patient outcomes.
Responsibilities- Manage and deliver patient care plans, collaborating with colleagues to ensure seamless care transitions
- Provide high-quality, patient-centered care, adhering to industry standards and best practices
- Collaborate with patients, families, and healthcare teams to achieve optimal patient outcomes
- Participate in on-call rotations, providing 24/7/365 support to patients and families
- Stay up-to-date with industry developments, best practices, and regulatory requirements
- RN license in the state of Michigan
- Associate degree or higher from an accredited School of Nursing
- Two years of RN experience, hospice experience preferred
- Current driver's license and ability to spend ~20% of your day driving to/from patient locations
- A commitment to consistently meet critical deadlines for charting
- The skills needed to self-manage your time and schedule
- Demonstrated experience with tablets, mobile phones, and EMR software
