Job Summary:We are looking for a dedicated and experienced Home Health Physical Therapist to provide physical therapy services to patients in their homes. The ideal candidate will have a strong passion for delivering exceptional patient care and a commitment to excellence in their work.
Responsibilities:- Conduct comprehensive assessments of patients' physical abilities and develop personalized treatment plans to improve their mobility and function.
- Provide one-on-one physical therapy sessions to patients in their homes, using a variety of techniques and modalities to promote healing and recovery.
- Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care and coordination of services.
- Document patient progress and outcomes, and communicate effectively with patients, families, and healthcare teams to ensure seamless care.
- Current licensure as a Physical Therapist in the state of practice.
- One year of experience in home health or a related setting.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a strong commitment to delivering exceptional patient care.
