About Achieve Rehab and Nursing Facility

We are a 140-bed skilled nursing facility located in Liberty, NY, dedicated to providing exceptional care and services to our residents. Our team is committed to delivering a high standard of care, and we are seeking motivated and reliable Certified Nursing Assistants (CNAs) to join our team.

Responsibilities
  • Provide personal care to residents, including grooming, bathing, dressing, and oral care.
  • Change bed linen and maintain grooming supplies in resident rooms.
  • Ambulate and transfer residents using assistive devices and body mechanics.
  • Record resident vital signs, weights, and measurements, as well as intake and output, food consumption, and other information.
  • Turn and position residents as necessary, and observe them carefully to report any changes in condition.
  • Participate in collecting data for the MDS and plan of care, and provide input to nurses for development of the plan.
  • Prepare residents for meals and transport them to the dining room, ensuring food trays are accessible and assistance is provided as needed.
  • Keep accurate records of food intake and output, and be alert for proper ventilation, temperature, light, and noise control.
  • Promptly answer call lights and assist with other resident needs.
Requirements
  • Current Nursing Assistant certification from the state of New York.
  • Current/active CPR certification, preferred.
  • At least one year of Skilled Nursing experience, preferred.
  • Effective verbal and written English communication skills.
  • Highest level of professionalism with the ability to maintain confidentiality.
  • Ability to communicate at all levels of organization and work well within a team environment.
  • Customer service-oriented with the ability to work well under pressure.
